CLASS OF 2026 GRADUATES AND THOSE APPLYING FOR SCHOLARSHIPS MUST COMPLETE THIS FORM.

Graduate and Student Recognition Sunday is June 28th. Invite your family and friends!

In order for scholarship funds to be released, we will need proof of enrollment for the 2026-2027 academic year. Please submit a copy of your fall schedule or any other evidence that you are enrolled in to the church office by August 31, 2026.

The scholarships will be split into a Fall & Spring payment. Verification for the spring enrollment is necessary to receive second half of scholarship.

Scholarship checks will be written to the school. Please provide your student ID number and an address for the school when you submit your proof of enrollment. We ask that you allow at least 10 business days from the time you submit verification to the time the check will be ready. You will be able to pick up the check during church office hours or we will mail it.

IMPORTANT:  We ask that you send a “Thank You” note to the church office within two weeks of receiving your scholarship funds.  We will forward it to the donors.

SMZBC Scholarships are awarded to ACTIVE members of Sixth Mount Zion Baptist Church.

The deadline for this form is Sunday, June 14th.
